Nursing assistants and Dr. Rabins review two video vignettes showing actual scenes filmed in nursing homes of physically violent residents. These include a man who violently objects to having his blood pressure taken, and a resident who throws his lunch tray at a nursing assistant.

After viewing these vignettes, nursing assistants explain what could have been done to prevent these situations and what can be done in response to violent outbursts. Nursing assistants stress the importance of forming positive relationships with residents and knowing triggers which may cause residents to become agitated, frustrated, scared or violent. Strategies to respond to these situations are also suggested.

Nursing assistants and Dr. Rabins review two video vignettes showing actual scenes filmed in a nursing home of verbally violent residents. One man becomes uncontrollably angry when a nursing assistant requests that he doesn’t talk to her using bad language. He responds with rage, shouting insults. The other scene shows a different resident responding with uncontrolled verbal outrage. Staff suggest knowing what triggers anger in residents and avoiding such situations. Resident vignettes shown in this program document residents with resistive behavior. This behavior is often very upsetting to staff who are merely doing their job and want to make sure the resident is clean and dressed for the day and that appropriate medical information has been gathered. Nursing assistants explain their secrets of how they get even these most resistive residents to cooperate.

Anyone who has worked in long-term care, has come across a resident who calls out repeatedly because of dementia. After viewing two video clips of residents with this behavior, nursing assistants talk about the importance of recognizing this behavior as part of a disease. They also point out that calling out may some time indicate a resident is in pain, thirsty, lonely or have still another meaning. Their job, is to try and find out if there’s a reason for the calling out and if there’s anything they can do to make the resident feel more comfortable and secure.

It’s hard to believe that incontinence and other physical problems could be the triggers for abuse. But, the proof is a very disturbing testimonial from a resident who describes how he was punished, ridiculed and neglected because of an incontinence problem. Nursing assistants talk about this case both as physical and psychological abuse.

It’s only been recently that neglect has received national attention as a hidden but most significant form of abuse. In this program, residents no longer able to communicate and who require total care with all activities of daily living are documented with nursing assistants. These residents pose significant care challenges. Providing necessary care without the right frame of mind or being rushed could be a frustrating difficult experience, tempting a GNA just to skip over all or some of the care. Positive approaches to the caring for such residents are discussed.
